One of the most difficult things with addictions is that the addict almost always will relapse to using their drug of choice. Now some people think counseling is the option on an outpatient basis but that simply is not enough to change a person.

Because obviously the addict is not in the right mind to listen to reason. Counseling is a great tool for someone who used to be involved in drugs and needs to overcome the emotional problems that led to the need for a vice in the first place, but it is not going to replace rehabilitation.
Rehabilitation is an environment that represents separation from the drug of choice for a period of time long enough to change the overall state of mind. While the body might not be addicted, the mind is, and if the mind is, then it will simply lie to the user to rationalize the addiction. Often the only times a drug addict is able to get over their addiction fully is when it is too late, they had to spend time in a hospital to recover from the injuries inflicted by the overmedication of their emotional problems.
